It is never too soon for patients and families to at least consider hospice when a life-limiting condition exists. By calling us for a confidential visit, we can help your physician determine when it’s time to be admitted to hospice care.

Although there are specific guidelines for each condition, here are some general signs that might indicate it’s time to call Valley Hospice:

• Individual is confined to a chair or bed .
• Physician has repeatedly tried to adjust medications, but pain and/or other symptoms continue or are getting worse.
• Individual has had frequent problems with pneumonia, infections, and/or hospitalizations—patient seems to be getting steadily worse.
• Individual and family have an understanding with the primary physician that they no longer see the benefit of aggressive treatments.

We care for patients in whatever setting they consider home:

• the patient’s home
• a family member’s home
• a nursing facility
• an assisted living facility
• the hospital
• the hospice inpatient setting

Valley Hospice contracts with approximately 30 nursing facilities and all local hospitals. This makes it possible for patients and families to have the benefits of both around-the-clock facility care with a hospice focus.

When hospice is involved early on, the patient care team can fully address the physical, emotional, and spiritual needs of the patient, as well as provide effective emotional and spiritual support for their family and loved ones.
