Our Services
The Valley Hospice philosophy of care is focused on the entire person, not just their illness. Our care is administered by an interdisciplinary team of specialty trained professionals.
Hospice care focuses on pain and symptom control as well as the psycho-social and emotional effects of life-limiting illnesses on individuals and those who care for them.
With your doctor's help, our patient care team provides:
- Visits by Physicians, RNs, LPNs, CNAs, Social workers, & Chaplain's
- Pain and symptom management
- Crisis care nursing
- Personal care (bathing, etc.)
- Emotional and spiritual support
- Help with prescription drug costs
- Oxygen, medical equipment and supplies
- Nurses and physicians on-call for emergencies 24 hours a day, 7 days a week
- Inpatient hospice care in a dedicated hospice unit
- Caregiving training & help for family
- Emotional & social support for patient & caregivers
- Help finding financial, legal, and social services
- Spiritual support from a chaplain in coordination with family's own religious affiliation &/or beliefs
- Dietary guidance
- Physical, occupational, speech therapy if needed
- Volunteer companionship for patient & caregivers
- Connections Program for patients who improve: Monthly RN visits & contact with social worker & chaplain as needed; provided at no charge.
- Bereavement support

- To provide emotional, social, & spiritual support for you and your family.
- To help you be cared for at home, instead of having to go to the emergency room or stay in the hospital.
