Our Palliative Care Nurse Practitioner will make house calls, offering consults to assist local physicians in providing expert pain or symptom relief, and/or help with advance care planning, for patients and families confronted with chronic progressive illness.

Palliative Care consults are available to patients at the time of diagnosis, while they are receiving curative treatment, and through all stages of their illness.

When is it time for a palliative care consult?

  • Patient has just been diagnosed with a serious illness, needs help with deciding what the goal of care should be, or needs help in starting to think about decisions that will need to be made in the future.
  • Patient has unacceptable pain or other uncomfortable symptom (for example, shortness of breath, nausea) for more than 2-3 days.
  • Patient needs help with emotional or spiritual distress related to serious illness.
  • Patient has made frequent trips to the ER or has had frequent hospital admissions with the same diagnosis within the past 30 days, with little or no improvement in comfort or quality of life.
  • Patient would like assistance with being more comfortable at home, but is not ready or eligible for hospice services.
